i haven’t read the novel myself, and my earlier comment was based purely on how you described the situation but if Dr. prem was drunk and clearly resisted, then no—she shouldn’t be held responsible at all. in that state, she wasn’t in a position to give proper consent, and the fact that Pat still went ahead crosses a serious line.
even if Pat was emotionally overwhelmed or acting out of desperation, that doesn’t justify her actions. It’s important to recognize that what she did hurt Prem, regardless of her intentions. so while i understand Pat might be struggling in her own way, the responsibility for that moment rests entirely with her not prem.

i haven’t read the novel yet, but from what you told me that prem kept rejecting pat again and again, and then finally gave in while drunk i do feel Prem holds some share of responsibility. you're saying she didn’t actually do anything, and yet she’s the one being blackmailed. but to me, pat’s toxicity is on an entirely different level. that said, Dr. Prem isn’t completely blameless either she lowered her guard, and that choice had consequences
