I think Seo Jin said something about having flashbacks in the preview... He said the flashbacks felt too real, which is probably why he thought of calling Ae Ri.

If Court Lady Noh's doppelganger isn't alive could it be that she died a natural death? I think the scars only show up to those whose doppelgangers have been killed in either world, and I think it's location has something to do about the doppelganger's death. Lee Lim's scar shows up on his neck going to his face, and if I remember correctly, he killed his doppelganger by breaking his neck in Episode 1.
