That doctor doesn't seem to be an accomplce, let alone a mastermind of the kidnapping. That's why he was ready to help to the escaped ML at the hospital until the FIL called him.. He didn't know what was happening and wasn't aware about the ML being set up or even suspected. But I do think his actions (and actions of the father-in-law) could have indirectly cause the entire ordeal. There are some conspiracy (malepractice/crimes/unlawful experiments or something like that) in the hospital, the couple of perpetrators became probably victims of this, and he covers it up.

I don't think Kim Daemyung plays a psychopath. He seems to be vety much in love with his wife, he is very fond of her, and was very worried when the ML threatened her. He also would prefer to help her after that fall, but it was her who sent him into another round of chasing the main couple. It means he has emotional life, he has a circle of emotional connections,
But yes, sure, he is cruel and a serial manipulator, kidnapper and killer. He is also quite narcissistic which manifests in blaming others, "You made me do that bad thing, It's your fault." But he isn't totally narcissistic, not all the time. I am not psychologist, so I would say he is a narcissistic sociopath with pretty good impulse control, better than other sociopaths have. Sociopaths are usually not good strategists, but they can be brilliant tacticians (the FL in Dear X was a great example of it: weak impulse control, good-to-great improvisation and manipulation skills, focus on an immediate threat instead of strategic ones and so on). This guy is often good in strategy too. That's what makes his character dangerous.
No Man Hui was very slow as he was chasing the FL. He looked like he really has health issues with his legs. Eitheir both of them are victims of mistakes/malpractice/medical experiment gone wrong (remember they had a presentation about a new technology curing paralysis at the beginning of the drama). and we know the FIL who owns the facility isn't really scrupulous about rules and doctor principles, and he acted very suspicously about the ML getting access to patients' files (what does he hide?), or the couple is somehow connected to the death of the lead couple's daughther. We still don't know how the girl got her fatal injuries. Maybe there was some car accident, and both couples' children died, but the FL was physically fine, give or take, and the parents of another child got injured. so now they have been taking revenge.

I don't think the ML was cheating. The female doctor works in the behavioral unit, it was on the nameplate at the door of her office. The ML might have visited her as a therapist to deal with his family and mental/emotional issues, including death of the child, of course. Maybe it began even earlier when he was late and his wife asked questions. I think he did it unoffically, because he generally deals badly with his emotions and doesn't want to reveal it. It's very likely the female doctor has feelings for him though and may or may not give him some bad advices, whether it was intentional or not, to get him eventually.
