As others have pointed out, in My Mister it isn't a middle-aged person macking on an almost-child because there isn't a romantic/sexual relationship at all. In a similar way it isn't dodgy when a parent is older than a child or a sister older than her brother. I'm sorry you're struggling with the difference between types of relationship.

An alternative view would be that she's much too old to play mid/late 20s convincingly, and he's (slightly) too old to play a 19 year old. They've definitely picked the wrong actress if your interpretation of her supposed age is true.

She's 42. She has great skincare and is very pretty, but 42 is not late 20s or even early 30s. If the character is meant to be that young then I think they picked the wrong actress.

I don't think there will be 'romance' exactly - but there is an age-gap tag, so its likely that part of the female lead's 'revenge' against the woman who bullied her all those years ago, is to seduce that woman's young son. If you've watched Game Rak Ao Keun or Kenja no Ai it's a similar concept. Except that this is kdrama, not jdrama, so expect some 'accidental' falls into the ML's arms, a couple of umbrella scenes and lots of blank faced gazing.

I have taken issue with your characterising the seduction of someone's child - late teens or early 20s - for reasons of revenge as having anything to do with gender equality. Because it doesn't.

" Still, maybe this middle-aged woman waits until her former bully's child reaches 20 or 21 for 'gender equality' purposes. A middle aged man seeking out a former classmate's daughter in the same circumstances would be cool, too.

I'm also an older woman who doesn't enjoy 'noona' (and reverse M/F dramas) where the age gap is massive and/or the older person is in a position of power/authority over the younger. If people get romantically or sexually excited over the older/younger premise then fair enough, each to their own, but lets not try to pretend its some profound sociological commentary, please.

What is 'misleading' about it? It's about a relationship between a 40-something female teacher and one of the then-teenage pupils she taught 4 years earlier is it not? Someone with whom she went on private, one-on-one 'field trips' at the time. But, apparently, she doesn't start full-on slurping after him until episode 4. So that's OK.
