Uk only loved one woman and it was Cho Yeong a.k.a Naksu. Mudeok was just the name she used to survive and Jang Uk referred her as Mudeok to hide her identitiy. No one ever complained that he loved Mudeok because there was no Mudeok, she died when she was a child. Now he loves "Jin Buyeon" a.k.a Cho Yeong a.a.k.a Naksu because his soul wants her soul. They are just soulmates. And he finally let himself to love her in this episode because he doesn't want to die anymore. There were no two female leads. Only one. Cho Yeong a.k.a Naksu. The original Jin Buyeon was only a plot device.
