I think both dramas are similar in few aspects:
1. FL has terminal illness
2. ML and FL have a contract in the beginning but fall in love later on.
3. ML is supportive of FL, when learns about the illness
4. The ending is also similar - Being Positive

However, Smile to Life doesn't have any of the leads being a doctor (the ml is a musician)

If life could have other versions, and the "if" could be experienced, what would it look like? The drama realizes this "if" so that the heroine experiences two lives in parallel.
