Throughout his life, Nakamura suffered from multiple mental illnesses and health conditions, including alcoholism, bipolar disorder and paranoid personality disorder. As a result, he resigned from many of his roles starting in the late 90s. After a long hiatus, Nakamura returned to voice acting in 2011 but retired just over a year later. In June 2014, Nakamura's health deteriorated and he was hospitalized.
Nakamura died due to drowining while taking a bath at a Tokyo hospital on July 30, 2014. He was 79 years old.
