I’ve read the webtoon, and it’s still ongoing, but here are some changes I remember.
First of all, the FL doesn’t have a sister in the webtoon. She has three older brothers and is the youngest and only daughter. That’s also why she doesn’t tell her parents about turning into a man, as she believes they’d be heartbroken if they found out their only daughter became a man. She lives alone, and even though she has her bestie Yuri to go to for advice, she still feels pretty lonely.
There weren’t many details or flashbacks about Yunjae and Jieun dating when FL was still in her woman form. Yuri does have a crush on Jieun, but it’s not really explored much in the webtoon, so that subplot is just kind of an extra scene in the drama version. Also, FL’s sister is like an added character. I’m not sure about Yeongseok and the café owner; maybe they’re added too, or maybe they just haven’t appeared yet in the English translation.
In the Korean translation of the webtoon, the parents know about the transformation, and it’s said to run on the mom’s side. But they didn’t warn her beforehand, and they found out about Fl's transformation to a man much later, unlike the drama where they actually bothered to tell the Fl about it and also reassured her by telling her that she would eventually turn back at the end. So in the webtoon, Fl solely relies on her fate to turn back.
Kang Minju hasn't appeared in the English translation of the webtoon, so I'm not sure if she is a character in the webtoon or not.

I'm not trying to hate on anyone, or trying to defend anyone, but the kiss between Yuri and Jihoon at her fansign was also forced. Jihoon was uncomfortable and didn't want to kiss Yuri, but Yuri forced the kiss anyway so that the crowd would calm down and then complained that Jihoon didn't lose anything since she had already kissed Yunjae before and was mad because her kiss with Jihoon was her first kiss. I can’t even be mad at what she did, because the whole situation only happened because of that girl, Min-ah.
