In episode 5, it explained how Phoem became sterile. Basically, Shen's dad hired some thugs to beat up Phoem when he was on a work trip in Penang. They showed how brutally the thugs beat him up implying they were striking his lower half of the body. Medical intervention couldn't save his private part.
