Pretty Sure that the mom knows she’s not her biological child since she’s the one that rejected her biological child for having no spirit of veins and had them sent away. I think her resentment and bitterness over the fact that she herself or a child with no spiritual veins and somebody else poor child who had strong spiritual pains is what made her treat the female lead like she did.
