With that call from family he is jolted back to reality that he can never give EH what she wants but he is so madly in love with her that he is ready to grasp any straws in the ocean... in this case her falling ill and him being able to spend time with her... that's why he said "fall sick sometimes"
and she knows she is in love with him as well and that same story will repeat itself of hurt

In terms of character traits..
EH and ML make complete sense. EH and second ML will never gel well.
However, Hyeri is different. She is optimism personified. She is hope, positivity, confidence, motivation, an energy packet ... someone who loves herself and that's what 2SML needs... he has confidence etc etc.. what he doesn't have is someone who can show him the brighter side to situations and people... Hyeri does that..
ML and EH don't need someone who can show them brighter sides. They seek a companion whom they can completely rely on.. they are not people who don't get along with others.. ML is quite popular... with ML and EH it is purely about a specific pain/scar in life, dealing with it and the romantic love that they have and can't move on...
ML isn't toxic by any standards. He is quite sorted, caring, and very mature.
