1- I think the FL's mom and ML's dad have or have had an affair. Or at the very very least he has a big crush on her. That would explain why ML's mom, who is shown as very calm and self controlled, cannot even hide her contempt for FL's mom. 

2- I saw someone mention that the psychiatrist (2 ML) likes seafood, like the real Sae On. That would mean that the childhood story 2 ML told was a lie, or if not a total lie, he was the one in the mask? (BTW the use of the mask when they were kids is supposed to make us think of The Man in The Iron Mask right? ) Anyway, then who was the real orphan then? Sae On? He did seem a tiny bit interested in the name of that orphanage. 

3- I also wonder why the kidnapper keeps calling FL unnie, instead of noona... 

I did doubt the 2nd male lead when he told his story. It feels like when liars try to share stories of their atrocities but don't wanna get shunned, they resort to making themselves victims in the story.

I think one of the boys who went missing from the orphanage, "Park Minwoo" (they didn't show us his picture), is the current Baek Saeon.

 Grandpa Baek tried to kill him by drowning because he found out that the real Baek Saeon killed the other children, but the guy who owns the fish shop rescued him.

 Ji Sangwu might be one of the twins. He also found out that the real Baek Saeon killed the children, and Grandpa Baek tried to drown him. That's what the current Baek Saeon saw. (Ji Sangwu also remembered the design on the ring)

 Park Do Jae might be Ji Sangwu's twin, and he might think that Ji Sangwu is dead and that the real Saeun killed him, just like the other children. However, Park Do Jae doesn’t know that the current Baek Saeun is not the real Saeun. That's why he's taking revenge, because he thinks that Saeun killed his twin brother.

 On the other hand, Ji Sangwu is trying to find his twin through the project he's currently working on. Meanwhile, the psychopath (the real Baek Saeun) is working with Park Do Jae because both of their goals are to hurt the current Baek Saeun.

I haven’t read the book but I think the psychiatrist is behind the storyline.  He obviously has a crush on FL  and  could possibly be the real Bae SanEon.  Just because you are were an uncontrolled psychopath as a child doesn’t mean he hasn’t learned to curb his behavior as an adult.  The kidnapper is a patient he previously meet through his work And is using him to do his dirty work. But suddenly the sister shows up to cause trouble and Do Jae supposedly pushed her off the balcony.  So I think there are two different groups who are involved and they have different priorities.  Hee-Joo‘s mother what a piece of work she is but she is just a side show unless she is the kidnappers mother that is the reason kidnapper calls her sis.
