Hahaha seriously. I think what distinguishes this from many manga and anime that become live action dramas is that the entire premise of this storyline lies in the physical appearance of these guys. (of course it's not alone in this...who would've bought into live action Death Note if the Shinigami were portrayed by short men in costumes?)
1. We (the audience) have to buy into the idea that they are so hot/cool that they can get away with all their antics, that girls will become obsessed with them, etc. Regardless of the host club member, he must first and foremost be magnetically attractive to a large segment of the population.
2. In this drama, the characters all play characters that are primarily based upon their physical characteristics/appearance. The host characters that many of the club members create are inseparable from their bodies. They change their attitudes/personalities to fit a role determined by their physical appearance (twinness, cuteness, strong&silent type, sickeningly beautiful hafu/double, nerdy, aloof). This means that some of the characters can be portrayed by a wider range of actors, while some of them have roles that are largely determined by their innate physical characteristics.
That said, of course everyone in the cast is better looking than the average person because they are professional actors.
