That was the struggle the team had…how far to take the creative liberty in the drama and stray from the novel/history. There’s no historical records of Doek-Im’s love (except the few excerpts of them being happy and drinking/laughing together) and the novel didn’t have her verbalize it either so they couldn’t really have her do it in the drama without potentially offending some people. Myself, I felt San knew she loved him even if his words towards the end said otherwise.
After reading up on the history (Talking Cupboard had good summary), I think they both loved each other and were happy in their short time together. She was pregnant 5 time (just about every year) and he was with her almost every night with the exception of his time away from the palace and his prescribed consummation nights with the Queen, his duty. Jeongjo was a very disciplined largely unemotional King. He wouldn’t have been so obvious with his love for Doek-Im if she didn’t show him or let him know it was reciprocated but since there’s no record of it, we’ll never truly know. He practically lived in her residence until his death, using it as his library/gathering place and write to her each year on her death anniversary.
Regarding if a King could fake/refuse to be intimate with other concubines/the Queen, the King can do whatever he wants once he has an heir and is powerful enough not to fear his in-laws. From what some found in the documents, he power in court was stable so he would have only had children with Doek-Im had they lived. To prevent inheritance issues, he may have visited but probably stopped being intimate with the Queen. Hwabin was also largely ignored after the one pregnancy that never delivered a child. This was around same time as Doek-Im’s pregnancy with CP Munyo. Hwabin’s palace became like the “cold” palace.

With the exception of when she was grieving their son’s death, she did show her happiness with/to him in their marriage. It just sucked that they only showed parts of it in ep16 and in ep17 as memories. If it had been the 20 episodes then we could have seen more of their happy times together. She really only showed her sadness to the viewers and Court Lady Seo which was why San was caught off guard by her not asking for him or telling him to pass her by in next life.

That’s why I loved both their acting in here. In one of the BTS, Junho was discussing a scene with the director on his lines and he said, and Doek-Im just needs to give one of her expressions (I like you but…no lines). They were in sync to where it allowed him to naturally react and I can see it on his expressions (I know you feel the same, just admit it) in that telescope confession scene.
