Yeah, I get what you're saying about needing something more tangible. I guess the best way I can put it is this: take a what a woman can experience before and during her cycle and amplify it by hundreds—to the point where you essentially can't control or alleviate the symptoms without either someone else's help or an experienced understanding of what might at least mildly alleviate some of them. A rut/heat is something you take time off from work for, lock yourself in your bedroom and figure out what's gonna help alleviate some of these miserable symptoms you'll experience over the next several days—this is particularly difficult if you are an unbonded alpha/omega.

And when I said “uneducated,” I wasn't referring to someone who's new to Omegaverse. I was talking about an alpha/omega who hasn't been properly educated throughout their life about how to understand the signs and what to expect when they go into rut or heat.

I also added some additional details and clarifications to some of my other explanations.

Yeah, you got a pretty good understanding of it. Here's just a few extra details and some clarifications.

There are pretty clear-cut and commonly shared symptoms across most Omegaverse settings—increased body temperature, a significant intensification of natural scent and pheromones, a dramatic increase in libido, and a painful physical desire that can only be fully alleviated by another alpha/omega satisfying that need—and, in the case of usually only bonded partners, through the release of “comfort pheromones.”

Knowing what's happening is definitely going to depend on someone's knowledge base of heats and ruts. If you're uneducated about it, and especially if you have never experienced it, like Phatsa, then he's not going to have any clue how to read the signs. Nahkun, I'm sure, has a better understanding of omega heat, but probably not completely, since he's never had a long-term partner.

In this case, I feel like Nahkun asking Phatsa if he was in heat wasn't only because he's the consent king, but because he genuinely wants to feel wanted by Phatsa. He wanted to make sure Phatsa actually wanted him—and wasn't just being driven to want him by a possible heat. That's very common with caring alphas. In a situation like theirs, or when a bonding hasn't taken place, many caring alphas will restrain themselves because, even though an Omega in heat may be pleading with them to fulfill their needs, once the Omega has come out of their heat, they will quite often regret acting on that need because it was brought on by animalistic instinct rather than genuine desire.

I'm not convinced that was an actual full-on heat, though. Heats and ruts primarily last several days. I think what happened was brought on more by what Nahkun was doing in that bathroom spilling over into the bedroom lol. The more time they spend together, the more their pheromones become intertwined, and that starts influencing—if not outright dictating—what their bodies and desires need from each other.

Regarding, when and how someone would know their second gender... In most Omegaverse settings, people are born with their secondary gender, but it typically doesn't become apparent until puberty or adolescence. Most discover it through testing, usually done in their mid-teens and most likely mandated by the Omegaverse government—but that's a whole different conversation—while others present naturally through the development of pheromones or other secondary-gender traits.

Of course, there are always variables and influencing factors. Genetics can play a role in determining someone's secondary gender, but they don't necessarily dictate it. It's kind of like real life human genetics—just because your father's big nose was passed down from his father, who inherited it from his father, doesn't mean you're necessarily going get that same big nose 👃 😂

This Omegaverse is definitely showing me a few variations I haven't come across before. Secondary genders here don't seem to be common knowledge, and are definitely not regulated the same way I've seen in other Omegaverses. So again, it's all in the creator's interpretation.

Nahkin, from the hospital, calls Phatsa at the café and, while on the phone, tells him that Nahkun was shot—Phatsa is shocked. In the next scene, Nahkin tells Phatsa in person at the café, once again, that Nahkun was shot—Phatsa is shocked again. At least, this is how it went according to the subtitles lol
