Trust me on one thing. They are least bothered about US SC proceedings. Abortion is frowned upon almost everywhere in the world. The doctor did not want to commit infanticide as the baby was born before the full gestation period. He will be in trouble if word gets out.

When I said in my previous comment about "he doctor indeed is like representative of US SC" in my point no 3, I meant "like" . the doctor doesn't believe in abortion. Nanai would have to choose some illegal abortion clinic but he did not.

To answer your confusion points -

1. After birth, the abdomen is still inflated in women, the uterus takes time to move back and it never goes back the way it was before. As the baby was out of his lower abdomen, I am guessing he developed all the lady bits inside his body

2. My idea is that he got C-section. They did not show it on cam though. The first thing you may have seen the doctor moving behind him telling it would hurt was the epidural injection in the base of spinal cord to induce labour and make the lower body go numb. It is used on women.

3. The doctor indeed is like representative of US SC. He is what you mentioned as "a fanatical, ham-handed, anti-choice, anti-abortion dumpster fire" because that is the same thing told to women. Nanai is supposed to go through all the shit women go through. That is the point of this whole series. It is a revenge drama
