in my opinion: (1) xiao yao only ever saw him as a brother. when you view someone as family, you don't have any romantic thoughts of them. (2) xiao yao must've thought his actions/words were normal because they always shared a close bond since childhood. (3) cang xuan hid his feelings well enough for her to not suspect anything.
i think people tend to forget that these things are obvious for us viewers because we are given the perspectives of all the characters; it's not obvious for the character themself because what they know is limited to what only they've seen. let's say... if the drama never showed us cang xuan or xiang liu's perspectives, there would definitely be less viewers who knew that they had feelings for xiao yao until the "big reveal" scenes.
