Perhaps this is his mind stretching too far, or it could be the present-day Great in the hospital trying to correct his past mistakes to accelerate his connection with Tyme and avoid ending up on that hospital bed.

Here's my current theory: Great is reliving his past up until the hospital bed scene, attempting to draw himself closer to Tyme. I believe he has some control over the future, with his present self on the bed influencing his past self to connect with Tyme and correct his mistakes through better choices. As time progresses, it seems that both Tyme and Great are going to exist in separate timelines
