The English here was translated as "supposed" but it was more closely "nominal" twin brother meaning that because he was adopted into the family and they were close in age they were raised siblings and stated as "twins" with her being the older. But as he states, if you actually even thought about their ages HE should have been older than her in reality.

It's not a huge issue in "my" mind at least, just that originally the conversation is stating that he didn't even know the person's gender and then the "ta" was used with the subtitle for "ta"/she and they translate it as she in English as well. which again would make sense because the story is referring to the body being the one to have saved him and the body IS female regardless of whatever the gender of the original person who was transmigrated into the body
