It's a real job and that division actually exists within the MPD. They're a group of ten-odd people and they help in the arrest of ~100 people per year. They actually do look for people at crowded places such as Shinjuku Station in similar fashion as depicted in the drama.
I don't think that job is common in the west. There is a recognizer unit in Britain, but they work with camera footage and don't actually wander the streets. However, that's a different profession and does exist in Japan as well.
I agree that the twist is predictable, but it's not that important of twist to begin with. This drama is less about who and more about why, if you know what I mean. It portrays the complexity and fragility of what makes us who we are; and how easy it is to exploit the latter.
Everyone in this story had reasons for their actions, both consciously and unconsciously. Very strong and deep character portrayals for the most part.
Great review! WOWOW produces some of the most gripping shows in Japan and doesn't shy away from using fresh talents behind the scenes.
