He faked his death saving the emperor to get bonus points for his wife and child. The treatment was dangerous and untested so he didn't know if it would really work and he didn't want to get her hopes up which is why he wanted to keep it from her. He also didn't want her to wait expecting his return. In the novel, the healers who had experience with treating that specific poison hadn't had experience curing anyone who had been sick as long as he had been. They really didn't have a lot of confidence.

SFL didn't transmigrate from the real world, she is a character in a transmigration story read by our FL who is from the real world. SFL transmigrated from a fictional modern world. I think it is to be determined whether SFL has a real mind of her own or not.
