Finally watched it. As a huge fan of the manga, I have to say I'm not disappointed. There are some changes from the manga and Kyouichi is a little bit different from what I imagined, but overall I'm quite happy and enjoyed it. And oh man, that's one of the best M/M sex scenes I've ever seen in Japanese BL drama. I really have to applaud Narita and Ohkura for their professionalism and dedication, both are popular actors and they did amazing job in this.

Onii-san is only used to address older brother and never used for non-family related older guy. Unless of course, it is sometimes used by "entertainment worker" to address their clients, which is totally different matter. But a girl usually will never use onii-san to address a guy she's trying to hit on, it will be very weird. Senpai is also a very common thing, even a guy can use this term to address his senior. But usually more used in school/college environment, in workplace you just use "-san" to address older colleagues. So whether it is being sexualized or not, it really depends on the intention of the speaker, but in general both are very common terms with no sexual connotation.
