During ep 1 they attend the same party. The ml sees the fl and falls in love at first sight. The fl doesn't notice the ml at all.
After the party, a friend of the ml makes a mistake, confusing the fl's name with another woman. The friend then calls the ml and informs him that the fl likes him and wants to meet him. The ml is overjoyed and goes to meet her. On the way, he gets into a crash and damages his hand, leading him to lose his future as a surgeon. The fl never contacts him or follows up with him, making him mad and upset, especially when they meet again years later at work and she appears to have zero memory of him.
...But it was never her in the first place. The ml's friend just confused the names...

Also, slightly off topic, but your description of the sml reminded me of a side character in the korean bl 8'th sense. The ML there was rich, but had a very difficult and sad backstory, with severe emotional and psychological issues that he was dealing with at the start of and throughtout the development of the series. The side character i'm referring to was a "friend" of his who envied his wealth and couldn't see anything beyond it. When the ml was suffering from an emotional collapse, that "friend" stabbed him the back and did his best to destroy the ml's future. Truly awful.
People with such a mindset can't be trusted. They are toxic, as you said, and also dangerous.
not trying to be rude and anything, just genuinely curious. yours is the third or fourth comment i've seen asking about age gap. can i ask why? till now ive only seen questions about genre, style, whatever, and now age gap has become a deciding factor in watching a show?
Sorry i didn't really go through the cast list. From the looks of it, and the comments i've read on here from novel readers, the story is heavy on fluff and light on angst or intrigue so i just dived in for the fun without looking twice. so far its been really sweet and im enjoying it.
