Ji-hwan and Min-hwan are legal twins, but not related by blood. Both may have had young love crushes on Yeo-ri (they were all highschool/early college at the time), but there's no indication that she reciprocated.
Re: the yacht accident. We've seen the boys get into an argument, but nothing to indicate that Min-hwan caused the accident. Thus far we have seen Yeo-ri fall overboard and Min-whan rushing to the spot - but Ji-hwan (thus far) not on the scenes.
Finally, the Suho (수호) necklace means guardian/protector and I also thought it saved her life. But Ji-hwan's mother said it protected him from seeing ghosts - and Ji-hwan who didn't always wear it, scoffed at the idea. So, I'm not sure we will get a clear answer on what powers that necklace actually provides the wearer.

Well, you identified the one big gap in the theory. Because of the various surnames, it works only if Gang-uk mother is sister of 2ML mother. But somehow there is a very strong bond between Grandmother Paeng and the mother and I don't believe it began with the heart donor/transplant. I have absolutely hated the idea that Gang-uk was the recipient of some random dude heart donor that "just happened" to be a close friend (perhaps 1st crush) of Yeo-ri. HATED the idea of fate, etc.
